HOURS:

• Monday - Thursday   8am to 5 pm
• 2nd Wednesday       8am to 7pm
• Fridays                      8am to 4pm

REQUIREMENTS:

• Current dues receipt
• Photo ID
• Last lay off or termination slip
• OSHA 10 card &/or OSHA  T&D 10 hour class completion card (dated within the past 5 years)
• First AID/CPR card

SIGNING:

• To get on the Books; must be done in person with ALL the requirements listed

RE-SIGNING:

• To maintain your name on the books; must be done on OR before the second Wednesday by 7pm of every month via Fax (702) 457-7441, email to office@ibew396.org, or in person
• If this is not done, you fall off the books & must start all over again in person.
